hereby give permission for my child

participate in the following activities while in attendance in all Child
Care Programs operated by Bright Beginnings.
Pool Release
I give permission for my child to swim in the pool during swim time hours. I
will provide a floatation device if needed for my child.

I give permission for my child to be transported to an from field trips / swimming by Bright Beginnings

I give permission for the Bright Beginnings staff to apply sunscreen (which I have provided) to my child
